Insurer rating lookup

Enter any insurance company to see its reputation grade, NAIC complaint index, claim-denial score, and prior-authorization record.

Highly rated insurers

A- and B-graded carriers on PlainInsurer's peer-relative composite, not a personalized recommendation.

Highest-complaint insurers

These carriers received an F grade on PlainInsurer's peer-relative composite of complaint index, denial rate and prior-authorization data, not proof of wrongdoing.

How This Tool Works

PlainInsurer grades insurance companies on an A through F scale based on:

  • Complaint Ratio - NAIC complaint volume normalized by premium volume (Market Conduct Annual Statement data)
  • Claim Denial Rate - CMS Transparency in Coverage data showing how often claims are denied
  • Prior Authorization Score - CMS Medicare Advantage data on prior auth approval rates

All data is publicly available from government sources. See our methodology page for details on scoring calculations.

Every figure on PlainInsurer comes directly from NAIC MCAS and CMS Transparency in Coverage data; no editor types in numbers. See our editorial standards or methodology, or report an error. Grades measure only the published complaint/denial data; we don't recommend any insurer or policy.
